We Can Help

Our admissions team will obtain all necessary medical records and verify funding. Whenever possible, admissions liaisons will conduct an in-person visit to meet with the individual and their family, observe therapies and carefully assess each individual’s circumstances to determine program eligibility.

Start Your Journey

Pending clinical and financial approval, QLI coordinates with all parties to arrange the transition date and transportation for eligible clients.

Admissions Criteria

QLI serves individuals with brain injuries, spinal cord injuries, chronic pain, or other neurological or physical disabilities. Our program generally serves individuals who are 18 years of age or older, however the Admissions Committee will consider younger applicants on a case-by-case basis.

Each applicant must be medically stable and a ventilator must not be in use for life support.

Each applicant will be evaluated to determine their service needs, potential for growth, and the ability to benefit from QLI’s Tri-Dimensional programming. Individuals are accepted for admission without regard to race, religion, creed, sexual orientation, or gender. All clients are admitted to the program under the care of an attending physician. Funding confirmation and an opening in the appropriate level program is necessary prior to admission.
